Access to Care and Digital Technology Adoption

Providing non-acute care, which includes long-term care, home care and other non-emergency medical services, comes with its own set of challenges. These challenges can vary depending on the specific type of care and the healthcare system in question. In this eight-part Navigating Challenges in Non-Acute Care webinar series we will discuss the challenges of care access and digital technology adoption, and how health centers, clinics and other non-acute care providers overcome these challenges.

Speakers

Natasha Ramontal, DHSc – Moderator
Digital Health Strategist, HIMSS

As a registered nurse, with experience from the frontlines to executive leadership, Dr. Ramontal has created clinical workflows focused on increasing access to healthcare via technology, trained providers and their healthcare teams on the goals of the Accountable Care Organizations (ACOS) and provided viable technological strategies to assist providers with patient care. Dr. Ramontal has navigated through a number of clinical roles such as case manager, telephonic nurse at a large insurance company, home health nurse and as a nursing professor, creating a course at Caravan Health, navigating through the value-based care system for population health nurses. Dr. Ramontal is also a Certified Health Coach who has worked to improve health outcomes by training physicians, their staff and hospital staff on optimization to improve quality and community health outcomes utilizing a population health strategy.

Roli Dwivedi, MD
Clinic Chief Executive Officer, Community-University Health Care Center

Dr. Roli Dwivedi is an Associate Professor and the Mac Baird Vice Chair in Community Engagement and Advocacy for the Department of Family Medicine & Community Health in the University of Minnesota Medical School. She serves as the Chief Executive Officer and interim Chief Clinical Officer at the Community-University Health Care Center (CUHCC). Dr. Dwivedi’s interests include global health, culturally-sensitive care, improving health disparities, and mentoring medical students and residents.

Kim Ngan Tran, PharmD
Pharmacist/Pharmacy Director, Community-University Health Care Center

Dr. Kim Tran is the Director of Pharmacy at the Community-University Health Care Center (CUHCC). A practicing clinical pharmacist, she leads CUHCC's medication therapy management (MTM) and vaccine programs. Dr. Tran's interests include remote patient monitoring, the role of community health workers in addressing barriers to care, reducing hospital readmissions, and mentoring the next generation of pharmacy residents in community health.

Eric Maurer
Chief Innovation & Strategy Officer, Community-University Health Care Center

Eric Maurer serves as the Chief Innovation & Strategy Officer at the Community-University Health Care Center (CUHCC), a federally-qualified health center in south Minneapolis, Minnesota that is transforming care and education to advance health equity.
