Corewell Health Earns Davies Award for Using Clinical Informatics to Improve Care

Corewell Health has been recognized as a recipient of a 2024 HIMSS Davies Award of Excellence. Corewell Health, based in Grand Rapids and Southfield, is the first health system in Michigan to receive the Davies Award since 2018 

C.aring for patients across the state of Michigan, Corewell Health submitted evidence-based case studies demonstrating applications of clinical informatics to improve the care and wellness for their underserved and chronic disease patient populations.

Leveraging tools within their electronic health record to identify, empanel and case manage patients, Corewell Health launched an innovative high risk underserved clinic care model. Targeting patients with a high risk of inpatient admission, the high risk underserved clinic significantly reduced emergency department admissions and improved diabetes control while reducing medical loss ratio (MLR) for Corewell Health’s risk-based contracts.

In addition, Corewell Health highlighted its longitudinal care management model, enabled by an innovative care management platform that guides care managers through addressing clinical and social determinants of health needs. Since the launch of the longitudinal care model, the 608 diabetic patients who graduated the program demonstrated an average 2.24-point A1c reduction and (for those empaneled patients who also had hypertension) an average 16.33-point improvement in systolic blood pressure.

Finally, Corewell Health leveraged predictive analytics and patient indicators for target populations to identify patients at the highest risk for readmission. Leveraging these insights, Corewell Health deployed coordinated resources to focus on those individuals with unpredictable recovery journeys and would most benefit from support of care managers. As result, Corewell Health reduced readmissions for the patients identified in the program by seventy percent.
